Have any of you done this kind of improvisational expeditions? Totally off the cuff, and actually used some details though, pre-organized, so to speak?

Back in the day -- the 1E day -- we used to improvise adventures all the time, but we didn't roll up a random dungeon from tables as we went along. That was something you did on a rainy day to pass the time.

With 3E, any improvisation tends to mean ignoring the rules.
