If it helps, I know from personal experience that antidepressants can have really extreme effects that cannot possibly be placebo-based. Before I was diagnosed with ADHD, my GP, trying to find a "fix" for my issues, put me on venlafaxine (aka Effexor in the US I think), and jesus god that was bad! It's sad because I was pretty positive about it, and expected it to actually help, if in a subtle way. But instead my head felt like it was full of tickly cotton wool, I could barely think, it got worse and worse as I took more, and I sure wasn't getting any less depressed or more functional after 3 weeks (quite the contrary), when I flushed the rest of the pills and refused point-blank to take any more ever (I was 19-ish and living at home at the time). The GP wanted me to continue because the symptoms "might stop" but come on man, there's a limit. I was also becoming significantly more clumsy/less dextrous, which was er... not great. So I don't think it's really likely positive effects can all just be placebos if negative ones can be so real. I do suspect they probably need to be more tailored to individuals than they actually are though. Also, aside as it's a different kind of drug but wow, ritalin 100% not a placebo in my experience because the immediate and huge gain in function that gave me was astonishing. And if I don't have it, even if I thought I took it and just forgot to, the effects are obvious. Like, I'll be like "Why am I so forgetful? Why am I walking slow?" and then if I check where I put the pill-foil, it'll still be there lol. I was prescribed that when I was 20 and was quite skeptical of it after the venlafaxine but damn. (As an aside, when re-reading after a while, I can tell posts I wrote without it lol I am significantly more of a jerk when I'm not on it!)
