For most, its a prolonged journey of reflection (either afoot or via one of the Rivers) until they gradually realize they're no longer on the plane they were and have arrived elsewhere. Others simply concentrate strongly enough upon the desired belief/mind-set to effectively "type in" the desired planar-address and abruptly shift there. </p><p></p><p>Likewise, EVERY hex/square in every plane is referenced by a specific planar-address. Teleportation occurs simply by over-writing your existing address for the desired location's address. Not that any one in-game knows this of course which is why the spell relies upon the user's ability to clearly focus upon their destination (so the Server may best-guess the desire location's address; much like a "I feel lucky Google search")</p></blockquote><p></p>
