What defines a Good Challenge in your rpg games?

I'm listening to a podcast where they talked about what makes a good challenge in a role playing game and how different GMs might use challenges in different ways to make things interesting for their players and it was interesting how they went on about how different systems can either help or hinder people if they wanted to do certain types of challenges.

So I thought I'd ask all of you what makes a good/great challenge in your games and what unique challenges have you come up with for your players?

There are two kinds of "good".

There is "good", meaning that it was difficult, caused the players to stretch their characters' abilities and think creatively in order to get through in one piece and at least partially succeed.

There is "good", meaning fun, engaging, and interesting for the players.

The two are not synonymous.
