It moves you closer to your goal (killing the monster in combat) but it doesn't get you there in one roll. Similarly, a failure is like taking damage. That is my default style as that is how I learned to play in the mid '80s. I just want to point out there are other perfectly acceptable ways to handle it, and they can be fun!
