What is "Opposite" for flanking?

In the following diagrams X=Monster and *=Character.

(a) Which of positions 1, 2 and 3 would grant flanking:

O*O


(b) Which of positions 1, 2 and 3 would grant flanking:

O12
OX3
*OO

Officially, only number 2 in both cases...
In fact in this situation:

*O*
OXO
O*O

Following RAW nobody here is flanking the person in the middle, but I call BS on that...

Does it? I thought I read somewhere that if a creature is flanked in 4E, every enemy of the creature gets combat advantage. While in 3E it was only the flankers.

I could very well be wrong, just asking out of curiosity.
