Earth elementals and xorn aren't strictly allowed as cohorts, since they don't have a level adjustment, (cohort) or not.
I'm not looking to go outside the rules themselves -- I was curious if anyone recalled any [earth] cohort critters from MM2, MM3, or Fiend Folio ...
Let me see here. I've got all three within arm's reach, so let me check.
Stone Spike from MM2 might not be a bad call, though there's not really anything to distinguish it from a regular earth elemental. It has two attacks at 1d8+4 rather than the earth elemental's single slam for 1d8+7 (and doubled Power Attack damage). That's pretty much the only difference between the two critters.
Nothing much from the other two books that would fit. All the [Earth] creatures are either too powerful (10+ HD) or not really the type of thing you'd want to have following you around (Avalancher and Dust Wight, I'm looking at you two. Knock it off. Seriously.)
The Miniatures Handbook has a few possibilities.
The Magma Hurler is fairly low Hit Die (4) and is listed at CR 3, but it has a DEVASTATING natural attack. With a 3d10+8+1d6 fire natural attack, it's doing quite a bit more damage each round than other creatures of its CR. 's a possibility, though.
A stonechild or earth genasi cohort would work well. It's not the most critter-like, but it would be a good, solid cohort to have around if you need somebody smashed in the head. Load him up with some full plate, a shield, and a bastard sword, and you have a literal brick between you and the bad guys.
A walking wall would be another fun idea. It's a defensively-oriented earth elemental that can harden itself to provide cover for its allies, as long as it doesn't take other actions.
