"Rewarding failure" is an issue when a player makes a really bad decision that isn't interesting enough to get a fudge pass, but when "failure" is "this little hunk of plastic rolled with a low-numbered face up" what exactly are you trying to punish?--4E's tendency to get out of the way is much of why I cherish it - I can focus on exciting story details and my terrible voice acting instead of niggling rules details.
