The main problem with aging is that of campaign time. In one shot or short term campaigns, it's a meaningless restriction. In a long term campaign, it might work fine, but let's say my first level wizard happens to be 80 years old; then it's too restrictive just because of a roleplaying choice I made.

I think the spell is actually fine as written except for the permanence of magic items and gold summoned; I just would like to see it expanded to cover some of the corner cases people are touching on.

How about a random table of possible drawbacks, like the Deck of Many things? :)

(without positive effects of course, just a chance of "no effect", some cosmetic/irrelevant or temporary effect, and some pretty major ones)

Example:

d20 - Effect
--------------
1-5 No drawback
6 You are fatigued/exhausted
7 You are affected by a Harm spell
8 You are affected by energy drain
9 You cannot cast spells for the rest of the day
10 A random ability score of yours is reduced to 1 for the rest of the day
11 You are affected by a random disease or curse*
12 All your personal magical possessions are subject to a disjunction spell
13 Your alignment, gender or race changes permanently*
14 You permanently lose one point in a random ability score
15 You permanently lose one HD and 5 HP
16 You permanently lose a spell slot of the highest level
17 You quickly age into your next age category
18 You lose a level
19 You permanently lose one point in all ability scores
20 You are subject to a trap the soul spell


*DM's choice or randomly determined
