*kinda OT but at the same time kinda ON-T*
When I need to throw some epic level nasties at a party, it's so easy to take a high-level demon or the like and stack on some levels in Fighter, Blackguard, Sorcerer, or whatever and just chuck it on in there. Works every time. ;)

this one has a cold...
this one hates broccolli...
this guy over here has terrets syndrome AND fire breath...

no one else remembers any more unique epic-level beings? :(

Hrmmm has anyone looked in the Legends and Lore (AD&D) sourcebook?
Or even the original Dieties and Demigods (again AD&D)

I'm pretty sure there is stuff in there that hasn't yet been converted.
The only downside will be that these are probably copywrighted... :(
(but I'm not too sure how the copywright laws work anyway so maybe its a good place for ideas to flow)

Unfortunately, most of the epic level nasties are either covered by your list, or are PC races of some type with lots of levels.

I can't remember as far back as the 1st ed stuff (I was young, and my memory is like a steel sieve) and I don't have any of my older gaming stuff anymore. (well not D&D related anyway)

mythological characters cannot be copywritten, AFAIK they are public domain. therefore, i can do what i want with em ;)
