I agree -- there's no need for any mechanical changes: let it all be in story.

As a DM, you lose nothing, and you gain a player willing to change the parameters of the story he or she is telling.
As a player, you lose nothing, and you gain a memorable story that you will repeat for years because of your awesome one-eyed monk.

Everybody wins, and who knows, Gruumsh might show up with a special quest just for you...

Likewise, I wouldn't bother with any mechanical effect at all.

But then, the only way a PC in my game would lose an eye or an ear would be if the player wanted that as part of their backstory. And if that's the case, the last thing I want to do is enforce a penalty - I don't want to discourage people from making interesting characters!

(Nor would I allow them to trade a "one eye" penalty for an extra bonus somewhere else - my experience with merit/flaw systems has been distinctly negative.)
