Adjustments for CMB & CMD scores?

I'm doing some 3.5 to PF monster conversions and I want to make sure I'm familiar with all of the various CMB & CMD rules for determining bonus/penalty scores for creatures.

Can anyone let me know if I'm missing any other adjustment modifiers for any sort of creature type or other PF rules that give a bonus/penalty to CMB & CMD?

So far I'm aware of the following rules:

CMB = BAB + Str mod + special size mod
Fine –8, Diminutive –4, Tiny –2, Small –1, Medium +0, Large +1, Huge +2, Gargantuan +4, Colossal +8

Tiny or smaller creatures: CMB = BAB + Dex mod + special size mod

Incorporeal creatures: CMB = BAB + Dex mod + special size mod

----------------------------------------------------------------

CMD = BAB + Str mod + Dex mod + special size mod + misc mods (circumstance, deflection, dodge, insight, luck, morale, profane, & sacred)
Fine –8, Diminutive –4, Tiny –2, Small –1, Medium +0, Large +1, Huge +2, Gargantuan +4, Colossal +8

Concealment bonus adds to CMD score.
Penalties to AC apply to CMD score.

Any attack bonuses or penalties apply to CMB. Like a bard's inspire courage or the -4 to hit for attacking while prone, IIRC. A weapon's enhancement bonuses / weapon focus / etc... apply if using the weapon for the atack. Like with trip and disarm. Weapon Finesse likewise applies on maneuvers with a finessable weapon. I wish that Paizo had just dropped Agile Maneuvers in general and folded it into Weapon Finesse. :(

It does? I thought concealment was a %, not a numeric bonus?

Sorry, poor wording on my part. That was part of the notes I've been taking for myself as I was skimming the rules and it meant that you still roll for concealment miss chance when performing a combat maneuver. I didn't mean to confuse anyone. :o

So am I missing any other rules regarding creature types that work differently the way tiny/smaller and incorporeal creatures do (i.e., they use dex bonus rather than str bonus for CMB)?

I was wondering if there were rules like that for multi-legged or limbless (like Beholders) creatures CMB/CMD adjustments; but I couldn't find anything. Other than the specific attack, like not being tripped, or a grapple bonus. I'm just looking for full CMB/CMD adjustments.
 

Every leg beyond the 2nd gives +2 to CMD against trip. And probably some of the newer combat maneuvers. Creaures with enough legs like monstrous centipedes are just outright immune to tripping.

I think you're correct otherwise, though I'm still trying to figure it all out myself.

As for concealment...RAW I think you're right. But I would suggest a houserule that if you're already grappling someone, concealment doesn't apply on any further grapple checks you make against the target, or at the very least the roll to maintain the grapple! Cause once you've got ahold of the creature, doesn't make much sense that some fog or invisibility would cause them to slip out of your grasp!
