Savage Worlds is great, especially for low-magic/sword & sorcery settings. There are a few products for that genre too: Beasts and Barbarians, Lankhmar, and Solomon Kane to name a few.

SW is great for any low-magic setting really. The magic system is incredibly modular and not tied to classes (SW uses skill-based advancement) so it's really easy to tailor the rules so the setting.
The warhammer frpg rules (especially the 2nd edition) work great for that too. I mean the warhammer setting is great, but you can use the rules outside the setting.

I haven't tried this, but I would quite happily use Mythras for a Theros game, or for a more low-magic game in any generic setting. For more lyrical or saga feeling games I'd probably use a modified Dungeon World rules set.

It would require a fair amount of hacking, but Mage: The Ascension has a lot of tonal fit with Planescape (factions jockeying for their philosophy to take control of reality), and everyone being some kind of caster fits better with the setting, I think.

The last campaign I ran before going full in on 3e upon its publication was exactly this and I found it to be a really snug fit. I don't remember much on how we rigged the rules and such, it was over 20 years ago, but I don't think there was any major surgery. We just kinda figured what mage traditions aligned best with what Sigil factions and then just kinda squinted. I also remember working the D&D alignment system into the spell casting in some capacity, but I can't remember much how.

For Forgotten Realms, Greyhawk, Dark Sun, Ravenloft, and Al Qadim, I would go with Savage Worlds, Rolemaster, and, maybe, even Tiny Dungeons 2e.

See... I think Eberon could totally be pulled off with the Iron Kingdoms ruleset. WarJacks would make pretty intimidating "Warforged".
