Just how compatible is this with standard 5e and its many supplements? If you drop a vampire, lich or dragon from the 5e monster manual into this game how hard are they to convert? What about spells or magic items?

It's 100% compatible. You can drop in a Terrasque for your heroes to fight if you like, or bring your hero into a standard D&D game. There are some optional new rules systems to manage things like secret identity, team morale, time travel, collateral damage, and combo moves, but otherwise the rules are the same.
