I think it varies by game: I forget if WotC has come up with an alternative, but Pathfinder has something.

Looking into it, Pathfinder Liches now have "Soul Cages," while WotC kicked the can down the road when they stated up Vecna by making him regenerate without the aid of any object, but they acknowledged thar future takes on the Lich will have a different word.
