I remember Mearls or someone mentioning that this rule variant would be included in the DMG quite a while back. Can anyone who actually has a copy confirm whether or not this is in there?

A Fighter who was that concerned with maximizing his AC will probably also have Defensive Fighting Style, matching your Barbarian's AC of 27. And the Barbarian is also dependent upon a magic shield to achieve that.

On occasion? Monsters hit PCs all the time now! Almost nothing--not even the low-level monsters--has less than a +4/+5 to hit. I think they raised the Monsters' attack bonuses a bit too high this time around...
I think you should read the "Loyal Readers" section of Dragon Magazine #67 from November 1982. Gary Gygax pretty much all but comes out and says that D&D's default cultural template and set of assumptions is essentially European medieval fantasy.
