It's a chart you use when someone rolls a "1" to see how badly they fail. A d20 is rolled again and compared to the chart. The chart will tell you how badly the fail was ie damage taken etc. It's only used for casual play.

Looking to add some new flavor to our D&D campaign. My DM said she had an epic fail chart at one point and thought it would be great for us. Does anyone have one or know of where I can download one?
