3.5/Pathfinder for me. I haven't had any major issues with Pathfinder so far and it will allow our group to continue using the big stack of books we have already.

Gnomes and half-elves with half-orcs as a close runner up. They just really do nothing for me. None of my group has ever played a gnome or half-elf, and there have only been a few half-orcs here and there.

Rerolls for our group are for hp.
Max hp at 1st, greater than half the die at 2nd and 3rd, with one reroll of hp between lvls 1-10 and one between 11-20. Player must take the 2nd roll.
Its not much - just a little to make up for the occasional '1'.
Another vote for Thieves World and Black Company here. I think Thieves World is coming out with a packaged set of the three or four books or may have already done so. If you've seen Black Company and liked it you should like TW.
I'm playing a dagger wielder right now. Two levels of fighter, three or four rogue (haven't got this far yet), then back and forth in Invisible Blade and Master Thrower and finish up with rogue levels to 20th. Right now he's not the strongest, but those two prestige classes will definitely help...

For #1 a few that come to mind would be the Master Thrower and _?_ (dagger specialty - forget the name) from Complete Warrior I think (very Shadowspawn), but there are lots of others too. Magic using ones would have to be converted.
