So... what's the deal with health care in the States? If this happened in Canada, he would have all his medical care, his prosthetic, his physiotherapy, everything he needs paid for by the public healthcare system.

I would say anything that introduces a cool villain who will stick around for a good portion of the campaign. Maybe he starts off as a patron, offering missions before he's revealed to be evil. Or maybe he's obviously a bad guy, but is in a position of power and authority such that the PCs can't...

Mmm. Some feats are great... lots are overpowered. My experience is that 4 times out of 5 a player will take a feat over the +2 ability points, which to me indicates they're too powerful.
