For the elder brain if you want, you should look at one of the boss of neverwinter "Yshiggol". When I played It a while ago the fight was memorable.

Go to the first page of the thread and click on "5e Epic Monsters by CR table" you will see an epic bonus row you add that modifier to its ability score modifiers, AC, Attack Bonus, Save DC, and damage to each attack.

So does the Azathoth (at stage 10) regenerate 250 hit points at the start of its turn?

Love the stat block for Acererak that you did! just got the book that holds the tomb of horrors and it wanted me to use the normal stats block for demilich, so thanks for making acererak unique!

Have you thought about making stats for sess'inek? Because I thought he was one of the demon lords of the abyss.
