A good number of books are in fact printed in Canada by Quebecor. They have printing press in US but my understanding is the one in Quebec are much larger and busier. In fact, there is a lot of time that books are printed in Canada, then shipped to US, only to be shipped back to Canada again...
Well, that is a rule I have adapted to ever single campaign I have run. No patron deity, no resurrection. Yet to have a player object to that rule. Most have commented on how they like that it adds flavour to the campaign and a reason for their PCs to follow a deity or deities.
That was some mismanagement by TSR. I was working in a bookstore at the time and any novel from TSR flew off the shelves at least the fantasy ones. The Buck Rogers line just rotted on the bookshelves. A good line of Mystara novels could generated another successful revenue stream. Very sad.
