I think the intent is you are supose to completely lose anything you get from your heritage. You can work with your DM on improving it. It didn't come up in my campaign but my thing was do you lose the humanoid type in wild shape?

Considering its a reaction and the trigger for the reaction is "when you make an attack" I don't see why it would have to be before you see your roll. Dm interpretation always supercedes but seem RAW to me you don't need to declare it.
Idk i would have read it as you already added the bonus effect so getting it again doesn't add the bonus. Thats why some things are worded add damage equal to your proficiency bonus instead of add your proficiency bonus to damage so it can stack with proficiency. No benefits from the same...
Id say you can definitely take it more than once. Every thing else with that restriction literally says you can only take once unless otherwise stated.
