Doom lets monsters get Advantage on attack, impose Disadvantage on PC Save, or refresh/regen an ability, like a Breath Weapon.
DM gets a budget of Doom points per encounter, based on party level and desired difficulty, or even "just because" for narrative reasons.

In a lot of respects, it reminds me of Flee, Mortals!. Going to have to really dig in with someone with a better head for math to do a side by side and really grok which to use when.
Do you have the other KP creature books for comparison? I love their imagination and themes and ideas, but even with errata I find them a bit hit or miss for CR. Flee, Mortals! set a high standard in that regard.

Much depends on the nature of crits at your table. My monsters don't crit; player crits do max+1roll; example a 1d8 weapon becomes 8+1d8.
