In my GREYHAWK campaign i use a very simple template for magic items handouts 3x5 print
CLOAK OF ARACHNIDA
Magic Wondrous item, very rare (requires attunement)
This fine garment is made of black silk interwoven with faint silvery threads. While wearing it, you gain the
following benefits:
• You have resistance to poison damage.
• You have a climbing speed equal to your
walking speed.
• You can move up, down, and across vertical surfaces and upside down along ceilings, while leaving your hands free.
• You can't be caught in webs of any sort and can move through webs as if they were difficult terrain.
• You can use an action to cast the web spell (save DC 13). The web created by the spell fills twice its normal area. Once used, this property of the cloak can't be used again until the next dawn.
PRISM OF GREYHAWK
Wondrous item, very rare
This crystal was rumoredly enchanted by Mordenkainen to enhance the prism's ability to create dazzling colors. The prism can cast two spells, each once per day; color pray and hypnotic pattern (Save DC 13). Note that the prism must be used in the presence of direct sunlight. It cannot be used under moonlight, normal torchlight, or daylight on an overcast day.
THE RENVASH SPLINTER
Wondrous item, very rare (requires attunement by a cleric or paladin)
This wooden shard is said to be a splinter from the coffin of a notorious priest of St-Cuthbert named Renvash, who perished at the service of a knightly order called The Society of the Sanctified Mind, which focuses on ridding the world of evil psionicists in the mid 500’s CY.
You can use it as a dagger, gaining a +1 bonus to attack and damage rolls made with this it. It also function as a Periapt of Wound Closure which means when carried that you stabilize whenever
you are dying at the start of your turn and whenever you roll a Hit Die to regain hit points, double the number of hit points it restores.
You can also cast Spare the Dying at-will, Cure Wounds 1/day, Calm Emotions 1/week and
Legend Lore 1/month.
