This really takes the idea into a different direction. Possibly a good direction! It definitely gets me to rethink how to theme out a location for each eye. The scale might be too big for what I'm looking for, like an entire floating city! But maybe something like a "floating tower" over a massive gorge in the old forest outside of town.
I guess smaller scale would be a the world's first and only airship. Have the eye set in the masthead and allow it to telekenesis ray. So instead of cannons or catapults or whatever as siege weapons, the eye rays picks up boulders or even a house, raises them up to flight level and then drops it.

I can see this being near a town and strange things happen. Like objects randomly move or get rearranged. Doors shut randomly or a rope grabs a person. People think the town might have ghosts or a curse, but it is the power of the Eye "leaking" into the town, and the priest doesn't have complete control over it yet.
I like it, the Telekinetic eye causes doors slamming, furniture moving etc, the Fear one causes nightmares and people seeing things in the shadows that turn out to not be there, the Charm eye compels people to do something ‘random’ and they remember that they should be doing it but have no idea why, etc

I am really leaning towards the amulets around their necks to make it seem like a piece of jewelry the cult wears, BUUUUUUUUT I think you are both right, and at least ONE of them has to HAVE it embedded because that would be a very cool surprise! Like I might wait to reveal which eye a priest has until AFTER the PCs take the necklaces from him. A scary twist when the priest uses the eye!
you might have one priest be a surprise difficulty spike for your group by having both the standard amulet and an implanted eye to spring a whole secondary power set on them!

i struggle to think an eye with the power to slow wouldn't also have power to speed things up, i think if you take some creative liberties you could use the slow eye to go full chronomancer.

death or enervation eyes seem right up the alley to making a the cultist into a necromancer and/or a lich, creating undead or summoning ghosts.

sleep eye could go take them into a whole mental realm and the cultist is a dream lord thing, the group has to figure out how to work and exploit all the weird dream logic to escape before they can properly defeat the cultist
