    spirit companion defenses?

Your defenses == its defenses. (PH1, in the area about Conjurations) AFAIK, spirit companions are effectively immune to Area and Close attacks.

    Gnomes and Stealth

I think the "allies" rule would still apply. I'd definitely rule that the gnome must be trying a little bit harder than "I always stand behind the tall/fat guy."

    When does Tiamat act? [Spoilers]

It's really up to the DM at the time to how Tiamat works. There is a lot of room left open for interpretation. My take: On each initiative count, the specific head is chosen. That head gets a standard action and a minor action, and Tiamat's immediate action is refreshed. If Tiamat wants...

    When does Tiamat act? [Spoilers]

I would probably allow Tiamat a single move action per round, taken during one of the head's turns.

    Magic Garrote?

I've been wondering... Garrotes used in a grab are fine. That's how I would imagine a garrote being used. Garrotes used in normal melee? Err... Um... Do you try to cut off fingers? Shave off a bit of skin? I'm just not seeing it. :hmm:

    D&D 4E (4E) Swordbond Travel Times

Not that often, but it's fun to have a character built around it. Think Wulfgar with magic. :p

    Quick question abous power-swap feats

The rule is that you can not take the same feat twice (certain exceptions apply).

    Night Hag-- Save or die?

Unconsicous = Dead = Petrified for all the player cares. If you never wake up, you are effectively "dead." The results of the power do not magically go away after the encounter ends, even if the power itself does. You do not heal damage. You do not remove conditions that aren't explicitly...

    Night Hag-- Save or die?

That is a rule for powers, not a rule for conditions. If a power states that you are dead, you don't come back to life after 5 minutes.

    Night Hag-- Save or die?

When weird questions like this pop up, always check the Updates. The night hag was fixed in the latest batch.

How does DC errata in DMG affect published adventures?

Adventures written before the errata update have more difficult skill challenges (and probably traps) than the adventures written after the update. If you wish for the skill challenges to be "more correct," -5 to the DC would go a long ways. H1 was definitely written with the old DC's. I'm...

    Starting Alchemical Formulas

The easy way: Make them buy their own formulas with the gold given to them. A "fun" way: Give them 2 formulas for every 5 levels. So if you're level 21, give them 2 between 1-6, 2 between 7-11, 2 between 12-16, 2 between 17-21.

    Determining Disease DCs

I think that if using all the updates, Maintain is a Moderate check while Improve is a Hard check. (DMG 42) That'll get you close to most published diseases, I think.

    Domination and Opportunity Attacks

I rule in favor of the player. No OAs while dominated. You still get saving throws for hazardous terrain. Etc. It mostly boils down to not being a complete jerk to your players. For example, you get a saving throw if you are going to be forced into Hindering/Hazardous terrain, right? You...

    Unconscious (no save) ?? Question

Ah, okay. I'd always considered that more of a clarification or reminder than an error. Sort of like Combat Challenge and Divine Challenge state "The target is marked. <Explains what Marked does>." If you read it strictly enough, using those abilities will lead to marked targets taking a...

    Domination and Opportunity Attacks

I think the RAW answer is that being Dominated doesn't change who your allies and enemies are. So enemies would still be enemies, allies would still be allies. Therefore, enemies could indeed take opportunity attacks on movement caused by Domination (hehe). I personally think that's a bit of...

    Action Point timing

Just a little nit-picky clarification: You can use an action point at any time during your turn, not during the round.

    Unconscious (no save) ?? Question

I put the water in my stomach through the means of swallowing it, but I didn't say that I drank it? :hmm: Okay, that's partially lack of sleep, but what exactly is the difference? Stone-Eye Basilisk (MM 26) states "The target is petrified (no save)." That seems to be a pretty "explicit (no...

    Unconscious (no save) ?? Question

That's actually incorrect. There are plenty of monsters in the Monster Manual that do no end on a save, but instead end when a certain condition is met. Illithids (Mind Flayers) have one of them.
