I wasted way too many quarters playing Shadows over Mystara in the arcade when I was younger and promptly bought Chronicles of Mystara for both Xbox 360 and PS3 when it came out so I could play with my friends on both systems. The D&D stuff is mostly cosmetic, but it's still a really fun game.

I agree. Any system that rewards the aggressor in any situation by allowing them to automatically go first (whether it's before initiative is rolled or by starting initiative on their count) greatly rewards players/characters that have a "kill first, ask questions later" approach.

I wouldn't roll initiative until the enemy party actually teleports in. If neither side has any way of actually attacking the other (because they're miles away) then combat hasn't started yet and initiative isn't rolled.
