I use the Roll20 ipad app for face to face play. The only issue is that initiative in the 5e Character Sheet for Roll20 is attached to a token and there's no map in the app, so I can't use the initiative button. I have to roll it manually.

I would add a 5th suggestion. I think you're overthinking it. If just do a wipe and say something like "three months later, your traveling in the south sword coast when..." they PROBABLY wouldn't blink. That said, having a sentence of what your doing down there wouldn't hurt. "Three months...

If you haven't established where they are yet, you wouldn't have to move LMoP. Also, you could just have a conversation with the players and move it. Or just do the letter thing. As someone who has been responsible for catching those for a game company, it is SO easy to miss them. You often can't do them until the very last minute, as the page numbers are changing throughout the process, and when you're scrambling to post the final file and can barely keep your eyes...
