Monster Knowledge Check, Monster Roles? Minion or not?

We don't out right say they're minions without a high DC check. We always know what monsters are of the same type and might describe a minions equipment or actions as sub-par compared to the other enemies on the battlefield.

    NPC as a character

We had an NPC as a teammate in a small group that worked really well. Biggest problem was his lack of healing surges though (1 per tier). We'd use healing that didn't spend his surge to do multiple encounters between extended rests. Also as an NPC he had a better to hit and AC than the part...

    granted saving throws

Yes, only saving throws at the end of your turn advance conditions (death, petrification, sleep, ect...). I think it's in PHB2

    Mounts

There is a mount slot item Nightmare's Saddle that allows you to teleport with a Nightmare if you're mounted. The Mounted Combat feat doesn't allow it.

Is there anything that modifies bloodied and or surge value

I want to say there are plenty of them. :) There are some in the MM. I've avoided reading MM2 so I can be surprised when we fight stuff. So until I run again each fight may bring something new!

Do monsters do extra damage on a crit?

Of course you get about a 4-1 crit ratio with minions compared to a single creature too which is another reason to not add more damage to them.

    Divine Challenge/Sanction Question

There are two different ways you to handle it and both are RAW due to vagueries in how "attack" is used. I'd always gone with "if you can choose each target and don't always choose the marker then you're effected". They would be effected if you used Twin Strike (can be same target) but not...

    Flame Seed and Forced Movement

Just liked some opinions on rules. "Different. Awefull, but different."

    Radiant and Fire damage

The lowest resistance/immunity applies. You take 10 dmg "radiant and fire" resist 5 radiant, resist 20 fire = 5 dmg

    Radiant and Fire damage

DMG pg 7 Some powers deal damage that combine two or more damage types. The target must have resistance or immunity to all of the specified damage types to reduce or ignore the damage.

    Adent Champion. Rules lawyers required

    300% percent increase. 5% x 300% = 15%

    Mounts

We've allowed the mount to shift with you when using "mount and rider are same creature" rules. If they want the mount and rider to have complete individual turns then they don't share those effects.

Use a free action after a charge?

The free action can't take place after your turn ends. What allowed you to use the free action? That trigger that occured on your turn and your turn is over now. Might as well take your free action next turn, after all it triggered before and it just wasn't used. We don't allow it in our...

Use a free action after a charge?

Not in our game. Free actions, in general, are not written so as to allow them in the middle of other actions. Some are specifically written to allow their use to modify actions/rolls before resolution. Your action is "charge" and once you charge you can't take other actions (except by...

    Masterwork Armor

You'd be unbalancing the game if you don't give masterwork as treasure or allow them to buy/upgrade. Masterwork keeps the balance of to-hit vs AC scaling through levels. Without it PC's fall behind the monsters.

    AV2 Item power levels

Not if it's a "Property:". The problem with subliments is power creep. Gotta have nice powers/items to interest people so they buy the book. Problem with a lot of subliment coming out in a brief period is power creep turns into power sprint. We haven't seen anything yet that we'd disallow or...

    On Ettins and Ongoing Damage

It's still only one creature. We've always given them damage and saves once per round.

    Divine Challenge/Sanction with Invis

True, but the original version of DC alloed a paladin to challenge and run away. This was changed prior to release so the paladin had to stay and fight (even if it's at range). The designers clearly did not want a paladin to be able to challenge and hide.

    Temporary Hit Points

Nope. The only way to add THP is if they're Invigorating THP.

    Ongoing damage vs regeneration

3 schools. 3) They apply at the same time.
