    If you're using a computer I find bookmarking some good random generator sites (like random NPC names, or a token making site if you're going to use roll20 or whatever) a god send for when the players interrogate a random passerby, or wander off on a side trek.

    Custom Items in Backstories

    Late to this discussion - just one tip: don't be afraid to be up front with your players about all custom stuff (items, feats, etc) being in continual "playtesting" mode and subject to tweaking as the campaign goes on. You never know what combo etc they might come up with making something OP...

    Indeed the ready action sucking is a design flaw. It means silly things like a monster moving out from behind a wall, shooting, then moving back behind a wall, cannot be properly dealt with.

    I agree the 6-8 encounters thing is awkward to pull off and stuffs up a lot of the balance of 5e. As you suggest, if you as GM expect a lot of save or die, and you're running say 3 fights/day on average - use two manticores in a battle instead of one. Course this can make things rather swingy...

    I think the third point is probably fair enough, although you could easily caveat that - you can delay but spells don't get extended as a result.

    It only has a chance of failure if you call for an attack roll. Personally, I would rule an auto hit, just like I allow auto slaying sleeping foes after the other combatants are taken out. Makes sense.

    The sorcerer will quickly run out of 3rd and 4th level spells, especially if you run the recommended 6-8 encounters/day. The GM never runs out of monsters. Just shrug it off, and look forward to the next battle!

    oh I totally agree - I would prefer a no feats game, or at least recommend liberal rebalancing of a number of feats (in fact, my personal preference is "custom only" feats, using the existing ones as a guide). But, if the OP wants to avoid ASIs, I think just take them away as a choice.

    I agree Int is the main dump stat, but can be made more useful by giving extra languages, and using illusions from time to time. Or giving out free "clues" with an int check. I think all armour is more class based than anything, and med armour is useful enough.
