Technically, it only autobreaks if the movement results in the party member ending up outside the range of the grabbing power/effect. Not that this really speaks to your concerns about the ease of which players can get free, but it's something.

I really like the idea of a thread for tracking errata to the Red Box. I'm glad I'm not the only person that noticed that Dwarves appear to have lost their CON bonus, and that there's no place for Passive Perception/Insight. Can a mod please clean up all the non-errata rants from this thread?

These are all fine guidelines for encounter design, but the OP wants examples of existing, published tactical combats.

Somebody will have to take an explicit (minor action) active Perception check to avoid the trap. If their Passive Perception isn't high enough, they'll walk right into it otherwise....
