That was an early house rule for me. I made Int skill bonuses and constitution HP bonuses retroactive.

After suprise, we used individual initiative modified by dex. Then add spell casting time or weapon speed to get order of what was going on or you could move.

Not a TTRPG, but Starfleet Battles uses simultaneous movement. I could see using that and weaving in actions as part of the movement chart.
