    In 3.5 Eberron you could be a dinosaur riding Halfling. A 5E barbarian Halfling riding into battle on the back of a carnivorous dinosaur could cause morale failures in their opponents. Better still a horde of them.

    Pretty much the same, pets and pack animals are ignored in combat unless the situation calls for it.

    Because a magic item is in the DMG doesn't mean that it has to come into play. A DM should have complete control over the items in their campaign. If you want to keep bounded accuracy at 20 then it is easy to do so.

    D&D 5E Monster Tactics: How Ruthless a DM are you?

    I have always tried to use rational tactics depending on the creatures personality, wisdom and intelligence. Most creatures would rather run to fight another day. However, in 3.5, I have not hesitated to use coup-de-grace if it was a reasonable tactic by the opponent.

    I believe that errata is more likely to cover errata and typo's rather than be about rules per se. It seems about the only rule that is firm is roll a d20 and the higher you roll the better. Even then ice rolling is discouraged unless the outcome is in doubt such as in combat.
