    The ruling wasn't targeted at flaming sphere, it was targeted at mage hand (which previously had the rather ridiculous side effect of being able to block a square with something that could only move 20 pounds). Also targeted wall of fog and such effects.

    Divine Challenge already needs renewed each turn, so it never would last until the end of the encounter. You get to issue it once per combat. Edit: Ok, I've misremembered, forget all of this :)

    The current errata states that conjurations don't prevent people from entering their square. Flaming sphere damages adjacent creatures. Therefore (by some arguments which I don't personally agree with), flaming sphere is actually a donut and you can stand in the center without taking damage.

    Switching weapons

    It doesn't matter. There is no penalty for attacking with whichever weapon (main or off hand) you want. The only requirement is that at least one of the weapons you're wielding has the off-hand property unless you're a TWF ranger.

    Because OAs are Melee basic attacks. And Eldritch blast isn't. The point of it being a ranged basic attack is for it to benefit from the Warlord abilities that allow characters to take basic attacks out of turn.

    Almost all rogue powers use dexterity rather than strength, but as far as I know, all melee basic attacks are strength based. Ranged basic attacks are dexterity based unless they're made with a heavy thrown weapon. The basic rule for basic attacks is at the bottom left of page 216.

    If one reading of the rules results in OMG what a cool overpowering thing I can do and a different reading doesn't, I tend to read the non-overpowering version as correct. Wow, I can stack unlimited temporary hit points sure seems to qualify as the first case.

    It's a simplification, rather than figuring out all the magic items the party might have gotten and dividing them up. In addition, hand-picked magic items can be slightly more valuable than the random ones you might pick up from adventuring.
