I got the quad masterscreen. Second time it was used, picked it up folded out from both sides and one of the panels ripped at the seam from the bottom to about 3/4 the way up. Didn't tug at it hard or anything, just the torsion ripped it. Fixed it with some duct tape and it is working fine...

If you want a mechanic that is easy to deal with but still have a "sting" you could require the raising cleric to loose a level for casting the spell or drain 1 permanent point of constitution (not even Wish could bring it back) from both the cleric and the dead.

If the players don't care and it isn't causing bad feelings maybe it really isn't a problem. I wouldn't give him a cursed item or similar targeted punishment. If the cursed item was in a treasure horde and made sense being there--great, but otherwise I think it sets up a "you're picking on me"...
