Oh, there is no definitive answer. It has been a subject of debate that (wisely, IMO) McGoohan never settled. I think having Number 6's background be implied but nebulous is best.

I use indexes differently I guess. I find myself looking up entries like "overland movement rules" or "underwater combat rules" which don't really fit into a Glossary format.

I hope you are wrong. The addition of the Glossary will pick up some slack from an index, but there are other needs that a glossary cannot address.
