One of my pet hates in D&D is if a DM or a Player for that fact tries to back track what has just taken place. I say if something happens during your game then let it happen. It is a shame when a DM kills some player then finds some way to make so the player did not actually die after all...

It takes a while to get used to it, but when you do it's rather helps people to not go running around carlessly in a fight. I think grapple is actually more anouying.
