    My main issue with "Milestone Leveling" is that it discourages creativity. Instead of figuring interesting ways to change the world, the PCs will tend to stick to actions that are expected to reward Minestone levels. This often means blindly and sheepishly following storyline hooks.

    That's for proficiency. Monsters may have proficiency in a save but terrible stats for it (the Tarrasque has proficiency in Intelligence saves) or vise-versa.

    Lore Bard based on Intelligence instead of Charisma: would that create an imbalance?

    I think your change allows for problematic multiclassing like Abjuration/Life on a Hill Dwarf. Also, changing the main spellcasting attribute from WIS to CHA or INT is a nerf, and changing INT or CHA to WIS is a boost. WIS is clearly the most useful mental stat.
