Exacty my opinion :) Currently play a PsW with Sure Strike Impact Bracers of Striking and an Amulet of Mighty Fists +1. The amulet is just to give me some boost in attack and damage.

Yep, but as a DM I would rule that he could hit the one he's grappling with automatically, but would need to win an opposed grapple check to control his "aim" to hit others outside the grapple.

If you assume mithral armor weighs half as much as normal armor, just like all other mithral stuff, and see the armor check penalty from dwarven plate as a minor error, everything is ok :)

The description ... ... probably means they don't need a concentration check for their breath weapon, and other SU abilities, but only for their spell-like abilities because ... [also from MM2, page 13] So once again the descriptions are not as precise as they could have been ;)

I think a concentration check of 20+ is hard enough, and besides hitting the other grappler, the breath weapons direction should be random.

Dragons are NOT immune to stunning, it is not the same as paralyzation. Only class I know of is the Sohei from OA, they get immunity from stunning effects at lvl 5.

We use the houserule that the toughness feat automatically upgrade to the next, dwarf's, dragon's etc, when you reach the needed base fort save.

You did it correct.

When the wizard casts it through his familiar, he gets the hitpoints. Temporary hitpoints do not extend to the familiar I think. The vampire gets temporary hitpoints, the spell description says nothing about healing or positive energy.

Grappling the dragon is rather cool :) But remember that if he isn't pinned in the first round, your constrictor gets all the dragons attacks. In MM2 you can read which attacks the dragon keeps in a grapple regarding to the size of the grappling creature.

Mind Blast can be very powerfull too. I once played a paladin/telepath lvl about 4/8. With his cha boosted to 24 or something like that those blasts were deadly. I remember rolling an 18 in one encounter where I was second in initiative, all enemies in a corridor had to roll a will save vs 29...
