I think that the gnome generally belongs in the MM. If you consider that White Plume Mountain is all about an insane gnome, the "D&D" gnome does not have a track record of being a "good" race.

I prefer tough 1st level characters if that means that an NPC expert with all 10s for stats and average hit points has to get lucky to kill a 1st level barbarian with max hp and 18 con. I don't give extra levels or hit points, but I don't like them to be pushovers. But I also dig camapigns...
