There were rules for such occurences in Savage Species. Now, this supplement is still 3.0, but those rules have not been updated to 3.5E as far as I know, so they are still the last 'official' word on how such things are handled.

Actually, because the 3.5E DMG came AFTER the ELH, the material in the DMG supercedes and updates the epic level rules. ECL replaces character level - once your ECL reaches 21, you're an epic character, meaning you start picking up epic attack and save bonuses instead of class-based attack...
