I do something similar as several other posters here, but instead of gold, I add the levels of the magic items together for each character and try to find new items for those who have lower totals that the rest. Almost the same, just with other values (in the dozens instead of thousands).

In my opinion D&D and particularly 4E isn't very well suited for this kind of game. There are some really great suggestions above, but in the end I don't think you will be able to create the kind of tension, fear and atmosphere you want to have in a game like this. There are several other games...

I really love minions, but as most of the other posters have already mentioned, automatic damage makes them just too worthless, that's why they get an increasingly harder save against these effects as well. The second thing is that they usually present not enough of a threat regarding their XP...
