This is very similar to how I run it. There is a sort of gray market in magic items, for those with the money and wherewithal to trade in such things. Various skill checks can get you in touch with the right people, at which point you may or may not be able to track down what you are looking for.

I give xp per session, not adventure. If they fall a little short of leveling, then they don't level until after the next session.

As a DM, I wouldn't allow combat focus to kick in unless you were actually engaging in true combat. But that's just me, I suppose.

If you have Ride-By Attack, would that negate the AoOs from all 12 heads when making such an attack? If so, a mounted fighter might be very effective making strafing attacks.
