For my group, I would say that the hardest things have been: 1) just getting used to how multiclassing works 2) attacks of opportunity and when they are triggered I guess that's about it. We never had any trouble with 2e, but 3e is rather easier, I think.

There's a bunch of stuff on incantatrixes (and even incantatars!) in the 2nd Edition FR supplement, Cult of the Dragon. I don't remember it too well offhand.. I think they were the magic-leecher types mentioned earlier, rather than the counterspeller anti-planar types they apparently became.
