    I usually simply ban disjunction. It's a broken mechanic, usually resulting in a TPK. With the Chain Dispelling, it's really not necessary. You could, perhaps make it a better version of Chain Dispelling, targeting every spell effect and item in the area, or give it a +5 bonus to the roll.

    I'd consider it, but it's not an automatic. Those feats are a big price to pay, and losing knowledge skills as class skill is somewhat problematic. The familiar thing isn't a big deal at all. The abilities are nice, but I don't think they're so good that you have to take it.

    The FAQ answer seems reasonable to me, though it pretty badly nerfs undead. For my own game, I'd give the same answer in general and then use my best judgement for a small handful of effects that ought to stack with themselves (like a wight's energy drain). That's a house rule though.

    Can a ‘normally’ wood hafted weapon be made with a metal haft?

    I'd allow it for hand weapons. Some sort of reinforcement for pole arms. But the most critical house rule for me is that breaking the shaft of a wooden shafted weapon doesn't ruin the enchantment. Just put a new handle on it and you're good to go.

    Warblade (book of 9 swords) and duskblade (PHII) are the closest to broken. They aren't really all that far out of line though. Some of the complete series classes might be broken on the weak side as full progression classes, but they can all serve a purpose for certain character concepts.
