    Take a look at the DMG errata. They've as much as admitted that the basic structure of skill challenges as admittedly released is wrong.

    As another poster's already noted, combination of fighter marking with non-fighter attacks from race or other classes was indeed noted by 4e's developers.

    No. The ranger (or whoever) hits him in a painful point, like near an eye or inside his jaw, that makes the dragon jerk in the opposite direction in pain.

    Yes. If you want the damage to continue, you keep having to use divine challenge (a minor action each turn). EDIT: My interpretation, anyway, and what seems like the RAW even if it's not the RAI.

    It makes the sleep spell and similar effects a heck of a lot less attractive, though.
