Yeah, if you wanted your spell to function as-written, I'd say you need to change the duration from Permanant/Instantaneous to 1 min/lev.

I agree with Archade. The original feats you propose are too powerful, and they aren't on par with each other, balance-wise. I think you could make them slightly more powerful than regular feats, using the same mindset of the regional feats from Forgotten Realms.

I would use this as a viable alternative in a no-magic or low-magic campaign setting. In a normal magic game, I don't see it being that useful, but I don't see it as being that game-breaking either. I'd limit the number of heal checks you can make to 1 per day though. Or 1 per wound.
