Anyone read Sea Wolf by Jack London? Have them be some mundanes, then get lost in space and then rescued aboard some sort of vessel. The ship wont turn around and cant abandon whatever its doing so they have to join up.

Wouldnt continual flame be enough? (Visually at least...I mean, I guess swinging a red hot piece of iron is pretty scary.) Would it add extra fire damage?

You could, but the damage is so low it would only bother commoners-whislt heroic (or villainous?) characters are likely just to hold on because they're that awesome despite the fact their sword is searing their bare hand. And if 2d4 is searing hot....what does a scorching ray or a fireball do...

Its a raid in DDO but they have Boots of Anchoring which are required, because the fiends may try to banish you back to your own plane during the raid. Boots of Anchoring - DDO wiki So I would imagine you could have an item enchanted with a constant Dimensional Anchor effect or the like.
