The Fiendish Girallon or Giant Constrictor Snake (SMV) and the Celestial Dire Bear (SMVI) are fairly good combatants. So good in fact that my 11th level wizard is considered somewhat overpowered in the City of the Spiderqueen adventure.

Yes. A measly +1 if you allow its psionic powers to be used 3 times a day, more like a +2 if you allow them an unlimited number of times per day. Hope that helps.

Protection from Arrows tends to fall before GMW. I think a big part of your problem is the ridiculous duration of GMW. At the higher levels (7+), the spell lasts all day (might need extend), and if your cleric bought (or you buy your cleric) a prayer bead of kharma for 5,000 gp, you suddenly...

I love to play Liches, but I tend to make do with Humans instead. Humans are fun, especially when you get into the cultures avaible in the world you play in (for instance Svimozh or Kalamaran in KoK).

The Assassin's Handbook has Fast Escape, a 1st level Assassin spell that grants you a +30 to Escape Artist checks. Verbal component only (still need the DC 21 Concentration check, though). The Shadow Mage PrC (same book) grants your wizard access to the assassin spell list.
