    I like D&D, Saga Edition, and Book of Nine Swords, so it stands to reason I will like 4e. I like the idea of every class having a mix of per day, per encounter, and at will abilities. It sounds like it keeps in resource management and attrition without over-encouraging a short adventuring day.

    My reading is that the character died, and you allowed him to live on with a major injury due to a house rule that allows you to cheat death once, is that right? I think Constitution drain would be the best option. Perhaps 1d4 points or something similar.

    No. Save-or-die has so many game play problems that I'm quite happy to see it go. The diminished value of tactics and player skill in an environment with somewhat common save-or-die effects is enough reason to wish it gone. Now they just need to weaken critical hits.

    I would make it require either Dexterity 13 or 15 to fit with other d20 system feats (virtually all feats require an odd ability score rather than an even one). Other than that, it looks fine. I introduced a similar feat in my own d20 modern game.

    Can you Cleave with Lay on Hands?

    If you can't use lay on hands with the Cleave attack, you can't use it with Cleave, as the extra attack must be made with the same weapon as the attack that dropped the opponent.
